Cognizant TriZetto Data Breach Exposes Health Information of 3.4 Million Patients

TriZetto Provider Solutions (Cognizant subsidiary) disclosed an external system breach that resulted in the theft of personal identifiers and protected health information for 3,433,965 patients. The intrusion began on November 19, 2024 and went undetected until November 28, 2025; notification to affected individuals started on February 6, 2026. TriZetto engaged the security firm Kroll and is offering 12 months of single‑bureau credit monitoring; the compromised data elevates risks of spear‑phishing, medical identity theft, and financial fraud.
