A USAF investigation found that Airman 1st Class Jack Teixeira singly stole and shared Top Secret-TS/SCI US military intelligence documents on a private Discord server beginning in February 2022. The report attributes the prolonged leakage to missed warning signs, failures in supervision, inadequate access controls and reporting, and disciplinary and organizational reforms have been enacted across the 102nd Intelligence Wing.
