Cognizant’s TriZetto Provider Solutions experienced a data breach that exposed sensitive personal and health information for more than 3.4 million patients after unauthorized access to a provider web portal beginning in November 2024; the company detected the activity in October 2025, notified affected providers, engaged cybersecurity experts and law enforcement, and is offering identity protection services, while reporting no financial data compromise or known fraud to date.
