Verity Health System disclosed a phishing-related email compromise affecting three employee accounts that resulted in exposure of patient protected health information (names, treatment details, insurance numbers, billing codes, DOBs, addresses, phone numbers) and a limited number of SSNs and driver’s license numbers; accounts were disabled, affected content removed, law enforcement notified, and mandatory employee training instituted.
