Texas Health and Human Services Commission Notifies Additional Individuals Regarding Insider Wrongdoing Breach

Texas Health and Human Services Commission announced an update to an insider privacy breach, notifying an additional 33,529 people—bringing the total affected to nearly 95,000—after confirming that PHI/PII (including names, contact info, SSNs, Medicaid/Medicare IDs, financial, employment, benefits, and medical data) was inappropriately accessed between June 2021 and January 2025; the agency has terminated nine employees, referred the case to OIG for possible criminal charges, advised recipients to monitor for fraud (including SNAP benefits), and is offering two years of free credit monitoring while strengthening internal controls and fraud detection.
