Licensed professional counselor indicted in $2 million health care fraud scheme

U.S. Attorney’s Office announcement: Juan Martin Flores was indicted and arrested for allegedly submitting over 15,000 fraudulent Medicaid claims between 2018 and 2022, causing roughly $2 million in improper payments and involving unauthorized use of beneficiaries’ personal information; the case is being prosecuted by federal and state authorities and investigated by the FBI, HHS–OIG, and Texas Medicaid Fraud Control Unit.
