TriZetto Provider Solutions disclosed a breach that allowed unauthorized access to an insurance eligibility web portal that began on November 19, 2024 and was detected on October 2, 2025, exposing sensitive personal, demographic, insurance, and health-related data for approximately 3.43 million individuals; the company reports no financial data was exposed, no confirmed misuse has been observed, affected parties were notified and offered 12 months of credit monitoring, and law enforcement and external cybersecurity experts were engaged to investigate.
