TriZetto Provider Solutions Breach Hits 3.4 Million Patients

TriZetto Provider Solutions (TPS), a Cognizant-owned healthcare IT firm, disclosed a breach discovered on October 2, 2025 that impacted over 3.4 million individuals; exposed data may include names, addresses, DOBs, Social Security numbers, health insurance member numbers (including Medicare identifiers), provider and insurer names, and other demographic and health/insurance information. TPS stated no payment card or bank account data were stolen, has involved law enforcement and security partners, implemented additional security protocols, and is providing credit monitoring to those affected; the parent company Cognizant has a history of prior security incidents.
